Individuals are asked to complete a W-9 form if they provide services to another company. In most cases, W-9 is applied to independent contractors, consultants or freelance workers. The information you provide is used by the company you work for to prepare a report to the Internal Revenue Service (IRS). This report states the amount of income paid to you or your company.   3. Check one of the boxes for federal tax classification
  4. Fill in exempt payee codes (applied to accounts maintained outside the U.S.)
  5. Provide your business address (number, street, apt. or suite no.)
  6. City, state and ZIP code
  7. List account number (optional)
  8. Fill in the requester’s name and address (optional).

Enter your Tax Identification Number (TIN), add your signature and date the file. By submitting W-9, you are certifying that the TIN you're providing is correct and accurate.
